A $25 processing fee is required before records are released.
Our practice is currently transitioning medical records to UT Health. If you prefer not to request your records from our office at this time, you may wait until the transfer is complete and request your records directly from UT Health. Any fees or procedures for requests made through UT Health will be determined by UT Health.
